CONTRAINDICATIONS
Hypersensitivity to the drug or to any of the excipients. Pheochromocytoma.
WARNINGS & PRECAUTIONS
Monoamine oxidase inhibitors (MAOI) and COMT are the two major enzyme systems involved in the metabolism of catecholamines. Therefore, a combination of entacapone and a non-selective MAOI (e.g., phenelzine and tranylcypromine) would result in inhibition of the majority of the pathways responsible for normal catecholamine metabolism. Thus, patients should ordinarily not be treated concomitantly with entacapone and a non-selective MAOI. Entacapone may be used with selegiline (a selective MAO-B inhibitor), but the daily dose of selegiline should not exceed 10 mg. Drugs known to be metabolized by COMT, such as adrenaline, noradrenaline, dopamine, dobutamine, alpha methyldopa, apomorphine, isoetherine, paroxetine, rimiterole, isoprenaline, and bitolterol should be administered with caution in patients receiving entacapone regardless of the route of administration (including inhalation), as their interaction may result in increased heart rates. possibly arrhythmias, and excessive changes in blood pressure. Dopaminergic therapy in Parkinson's disease patients has been associated with orthostatic hypotension. Entacapone enhances levodopa bioavailability and, therefore, might be expected to increase the occurrence of orthostatic hypotension. Entacapone Should be given cautiously to patients who are taking other medicinal products which may cause orthostatic hypotension.
SIDE EFFECTS
The most commonly reported adverse effects with the use of entacapone were dyskinesia, hyperkinesia, hypokinesia, dizziness, nausea, diarrhea, abdominal pain, constipation, vomiting, dry mouth, back pain, increased sweating, insomnia, hallucinations, confusion, paranoia, taste perversion, aggravated parkinsonism, anxiety, somnolence, agitation, dyspepsia, flatulence, gastritis, dyspnea, purpura, urine discoloration, fatigue, asthenia, bacterial Infection and isolated cases of hepatitis with cholestatic features have been reported.
