DESCRIPTION Cloxacillin is semisynthetic penicillin of isoxazolyl penicillin group, resistant to penicillinase produced by staphylococci. It is stable to gastric acid and is given by mouth in the treatment of staphylococcal infection. 

COMPOSITION: ALCLOX 125 SYRUP: Each 5ml reconstituted syrup contains: Cloxacillin Sodium BP eq. to Cloxacillin 125mg. Colour: Sunset Yellow FCF. ALCLOX 250 SYRUP: Each 5ml reconstituted syrup contains: Cloxacillin Sodium BP eq. to Cloxacillin 250mg. Colour: Sunset Yellow FCF. ALCLOX 250 CAPSULE: Each hard gelatin capsule contains: Cloxacillin Sodium BP eq. to Cloxacillin 250mg. Approved colors used in the shell. ALCLOX 500 CAPSULE: Each hard gelatin capsule contains: Cloxacillin Sodium BP eq. to Cloxacillin 500mg. Approved colors used in the shell. 

INDICATIONS: Cloxacillin is indicated for the treatment of infections due to penicillinase-produced by staphylococci that are resistant to benzylpenicillin. It is used against gram-positive staphylococcus aureus in skin and soft tissue infections, e.g. abscesses, cellulitis, pneumonia, endocarditis, osteomyelitis. It is also used for the relief of variety of infections Respiratory tract infection especially the lower respiratory tract, chronic bronchitis & bronchiectasis, post-operative infections, E.N.T Infections & Surgical infections.

CONTRA-INDICATIONS: Cloxacillin should not be given to patients with a history of penicillin allergy or administered to neonates born of mothers hypersensitive to penicillin. Patients allergic to cephalosporins may also be allergic to penicillin. Cloxacillin is incompatible with aminoglycosides, tetracyclines, erythromycin & polymyxin B. It is resistant to penicillinase produced by Staphylococcus aureus.

DRUG INTERACTION: Co-administration of probenecid or disulfiram with cloxacillin may result in higher cloxacillin concentration. Chloramphenicol and tetracycline antagonize bactericidal effect of penicillin. Potentially Fatal: Increased hypoprothrombinemia effects of oral anticoagulants. It interacts with food which result in delay of absorption, in presence of food. 

PRECAUTIONS & WARNING: While administering cloxacillin in high dose care should be taken especially to patients with impaired renal function as there is a risk of neuro - toxicity and congestive heart failure. Disturbance of electrolyte balance may occur following administration of large doses. Increased liver enzyme value has been reported. Renal and hematological systems should be monitored during prolonged and high dose therapy, patients with syphilis may exhibit the Jarisch-Herxheimer reaction and should also therefore be monitored. A skin test for sensitivity may be used to determine those patients most likely to develop allergic reactions to penicillin. In children, caution must be taken when given to infants and jaundiced neonates. 

 SIDE EFFECTS: Sensitivity reactions may include skin rashes. angioedema, bronchospasm, serum sickness, anaphylaxis. A generalized sensitivity reaction can develop within a few hours or weeks of commencing treatment, including urticaria, fever, joint pains and eosinophilia. Other allergic reactions-include exfoliative dermatitis, maculopapular rashes, interstitial nephritis and vasculitis. Hemolytic anemia, leucopenia, prolonged bleeding time and defective platelet function can occur. Ora administration may produce diarrhea, heartburn, nausea, hepatitis and cholestatic jaundice have been reported. A sore mouth or tongue, and a black hairy tongue have also been reported. Supra-infection with C. albicans, other fungi or organisms resistant to cloxacillin may occur. 

ADVERSE EFFECTS: Adverse effect include hypersensitivity, urticaria, eosinophilia, fever, neutropenia & skin rashes. Gastrointestinal effect like nausea & diarrhea. Hematological effect like agranulocytosis, anemia and thrombocytopenia. 

OVERDOSE & TREATMENT: Symptoms of Cloxacillin overdose include muscle spasms or weakness, pain or twitching, pain in the fingers or toes, loss of feeling in the finger or toes. seizures, confusion, coma, and agitation. In case of overdose, seek medical attention immediately.
