Individuals with normal iron balance should not take iron chronically. Accidental overdosage of iron-containing products is a leading cause of fatal poisoning in children younger than 6 yean of age.
PRECAUTIONS
Discontinue use if symptoms of intolerance appear. Diabetic patients, patients prone to recurrent renal calculi, those undergoing stool occult blood tests and those on sodium restricted diets or anticoagulant therapy should not take excessive doses of vitamin C over an extended period of time. Give to pregnant women only if absolutely needed. Avoid concurrent aluminum containing antacids. Administer with caution to nursing mothers.
DRUG INTERACTIONS
Absorption of cyanocobalamin, from the gastrointestinal tract may be reduced by neomycin, aminosalicylic acid, histamine H2 antagonists and colchicine’s. Serum concentrations may be decreased by concurrent use of oral contraceptives. Parenteral chloramphenicol may attenuate the effect of vitamin B12 in anaemia. Folic acid supplementation has reportedly decreased serum-phenytoin concentrations in few cases. Large doses (greater than 500 mg) of vitamin C may cause false-negative urine glucose determination.
OVERDOSAGE
In acute overdosage zinc salts are corrosive, due to formation of zinc chloride by stomach acid; treatment consists of giving milk or alkali carbonates and activated charcoal. The use of emetics or gastric lavage should be avoided. High serum zinc concentrations may be reduced by using a chelating agent like sodium calcium edetate. Overdosage of selenium has been associated with loss of hair, nail changes, diarrhoea, dermatitis, metallic taste, garlic odour of breath, irritability, fatigue and peripheral neuropathy.
