PHARMACOLOGICAL CATEGORY
Antihistamines for systemic use.
PHARMACOLOGY
Fexofenadine is a selectively peripheral H1 -blocker. Blockage prevents the activation of the HI receptors by histamine, preventing the symptoms associated with allergies from occurring. Fexofenadine does not readily cross the blood-brain barrier and is therefore unlikely to cause drowsiness. It also exhibits no anticholinergic, antidopaminergic, alpha 1-adrenergic, or beta-adrenergic-receptor-blocking effects.
INDICATIONS
Fexofenadine hydrochloride is indicated symptomatic relief of seasonal allergic rhinitis and symptomatic relief of chronic idiopathic urticaria.
CONTRAINDICATIONS
Hypersensitivity to the active substance or to any of the excipients.
SIDE EFFECTS/ADVERSE EFFECTS
Uncommon: Antimuscarinic effects; blurred vision; dry mouth; gastro-intestinal disturbances; headache; psychomotor impairment; urinary retention. Rare: Anaphylaxis; angioedema; angle-closure glaucoma (in adults); arrhythmias; blood disorders; bronchospasm; confusion; convulsions; depression; dizziness; extrapyramidal effects; hypersensitivity reactions; hypotension; liver dysfunction; palpitation; photosensitivity reactions; rashes; sleep disturbances; tremor, Frequency not known, Drowsiness. Side-effects, further information: Non-sedating antihistamines such as fexofenadine cause less sedation and psychomotor impairment than the older antihistamines because they penetrate the blood brain barrier only to a slight extent. If drowsiness occurs, it may diminish after a few days of treatment. In adults: Elderly patients are more susceptible to side-effects. In children: Children are more susceptible to side-effects.
PRECAUTIONS AND WARNINGS
As with most new medicinal products there is only limited data in the older people and renally or hepatically impaired patients. Fexofenadine hydrochloride should be administered with care in these special groups. Patients with a history of or ongoing cardiovascular disease should be warned that, antihistamines as a medicine class have been associated with the adverse reactions, tachycardia and palpitations.
PREGNANCY & LACTATION
Pregnancy: Most manufacturers of antihistamines advise avoiding their use during pregnancy; however, there is no evidence of teratogenicity. Lactation: Most antihistamines are present in breast milk in varying amounts: although not known to be harmful; most manufacturers advise avoiding their use in mothers who are breast-feeding.
OVERDOSAGE
Dizziness, drowsiness, fatigue and dry mouth have been reported with overdose of fexofenadine hydrochloride. Single doses up to 800 mg and doses up to 690 mg twice daily for 1 month or 240 mg once daily for 1 year have been administered to healthy subjects without the development of clinically significant adverse reactions as compared with placebo. The maximum tolerated dose of fexofenadine hydrochloride has not been established. Standard measures should be considered to remove any unabsorbed medicinal product. Symptomatic and supportive treatment is recommended. Hemodialysis does not effectively remove fexofenadine hydrochloride from blood.
