PHARMACOLOGY
ZECID-20 is a gastric proton pump inhibitor. ZECID-20 is a substituted benzimidazole derivative that acts on the final step of acid production & therefore controls intragastric acidity irrespective of stimulus. It markedly inhibits basal & stimulated gastric secretion. Thus, it is antisecretory drug which is very effective for rapidly healing peptic ulcers & erosive esophagitis. Omeprazole decreases basal and stimulated gastric secretion independent of the stimulated. The reduction of Penta gastrin stimulated gastric secretion is dose-dependent with single oral dosage between 10 and 80 mg. The highest dose of Omeprazole produces nearly complete inhibition gastric secretion and these effects lasted for at least 4 hours. The duration of action of Omeprazole is also known to dose dependent and significant inhibition of gastric acid secretion is seen 72 hours after a single dose of 40 mg. Normal gastric acid secretion has returned 3-5 days after a single oral dose with repeated administration of Omeprazole increasing inhibition of gastric acid secretion is seen up to 7 days after oral administration. Studies in patients with peptic ulceration have shown that maximum. Consistent inhibition of gastric acid secretion occurs in most patients taking 20 - 30 mg. daily. Individual variation in response is greatest at dosage lower than 20 mg. Plasma gastrin concentrations rise in patient treated with Omeprazole for periods of 7 days or more and return to pre-treatment levels within 7 to 14 days of stopping treatment.
INDICATIONS
Duodenal ulcer healing: Endoscopic healing has been confirmed after two weeks treatment with omeprazole in open and controlled blinded studies. Healing rates at two weeks range from 50% (10 mg. daily), 3-69% (20 mg. daily), 73-78% (30 mg. daily) to 91-95% (40 mg. daily). At two weeks the trends certainly suggest a dose response relationship with healing. After four weeks treatment healing rates are between 90-100% when doses of 20 mg., or more are used. Gastric ulcer healing: Omeprazole 20 mg. daily healed similar numbers of gastric ulcers as ranitidine, but rates were generally lower than in duodenal ulcer (43%, 81% and 95% at 2, 4 and 8 weeks). Esophagitis: In a double-blind placebo-controlled trial, Omeprazole 20 and 40 mg. daily produced endoscopies healing significantly more often than placebo (81% against 6% at four weeks) and symptomatic improvement was equally impressive. Another study using ranitidine as comparator showed significant advantage to Omeprazole 40 mg. daily. Zollinger-Elison Syndrome: In patients whose acid hypersecretion is uncontrolled by H2-receptor antagonists Omeprazole has been shown to reduce secretion to < 10 mmol. h in almost all patients in dose of up to 180 mg. daily. Some patients have received Omeprazole for longer than 5 years and the median dosage requirement is between 60 and 70 mg. daily. Details of these patients are given by Lloyd-Davies and others.
Other indications
Omeprazole would be expected to prevent much stress ulceration found in acutely ill or burns patients. The rapid antisecretory action could also be useful in emergency anesthesia to prevent acid aspiration syndromes but more data are required on effects during pregnancy. it is plausible that anacidity, but data are awaited. In a recent group of 17 patients with resistant ulcers (failure to heal in 6 months on standard treatments) Omeprazole 40 mg. daily for up to eight weeks produced 1000/0 healing.
ADVERSE EFFECTS
