METVIC-SR USP 500MG
Each extended-release tablet contains 500mg of Metformin hydrochloride BP 500mg.
DRUG ACTION
Metformin is a biguanide which exerts its effect mainly by decreasing gluconeogenesis and by increasing peripheral utilization of glucose. Metformin decreases hepatic glucose production and intestinal absorption of glucose and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. It acts only in the presence of endogenous insulin.
INDICATIONS
Treatment of type 2 diabetes mellitus, particularly in overweight patients, when dietary management and exercise alone do not result in adequate glycemic control. Metformin may be used as mono therapy or in combination with other oral anti diabetic agents or with insulin.
SIDE-EFFECTS
Common or very common: Abdominal pain, anorexia, diarrhea (usually transient), nausea, taste disturbance, vomiting. Gastro-intestinal side-effects are also initially common and may persist in some patients, particularly when high doses are given. Rare: Decreased vitamin-B12, absorption, erythema, lactic acidosis (withdraw treatment), pruritus, urticaria Frequency not known: Hepatitis.
CAUTIONS & MONITORING
Can provoke lactic acidosis, determine renal function before treatment and at least annually (at least twice a year in patients with additional risk factors for renal impairment or if deterioration suspected). Pregnancy: can be used in pregnancy for both pre-existing and gestational diabetes. Women with gestational diabetes should discontinue treatment after giving birth. Lactation: can be used during breast-feeding in women with pre-existing diabetes.
CONTRA-INDICATIONS
Known hypersensitivity to metformin.
INTERACTIONS
Ketoacidosis - use of general anesthesia (suspend metformin on the morning of the surgery and restart when renal function returns to baseline). Renal impairment - use with caution with renal impairment increase risk of lactic acidosis Hepatic impairment - withdraw if tissue hypoxia is likely. Renal disease or renal dysfunction (adults' dose should be reviewed if eGFR less than 45m l/minute/1.73M2 and avoid if eGFR less than 30ml/minute/1.73M2; children suffering from renal impairment should avoid). Withdraw or interrupt treatment in those at risk of tissue hypoxia, or sudden deterioration of renal function, such as those with dehydration, severe infection, shock, sepsis, heart failure, respiratory failure or hepatic impairment or those who have recently had a myocardial infarction.
